Baggy Lower Eyelids in a 73-year-old Caucasian female with Fitzpatrick type 1 skin
Patient is not a smoker and has never smoked but grew up in sunny California on the beach most days growing up
Surgery Performed:
Modified lower eyelid blepharoplasty with pinch skin removal, lateral canthoplasty, fat pad reduction, fat transposition, CO2 fractionated laser, mesenchymal stem cells application to skin of face
Case Study:
73-year-old female who complains for “baggy lower eyelids which I have had since I was young and I have always hated them. You did such a great job on my daughter, I wonder if you can help mine. My daughter had the Hammock Lift and is delighted with the result. I would like something limited to my lower eyelids for the bags if possible.
Requirements of the Patient:
I don’t want to look like I had surgery. Please don’t make my eyelid slant upwards like I have seen in people after lower blepharoplasty. I would like the result to look natural as if I am just rested. My skin is also dull and I would like to improve it at the same time if I can
Clinical Assessment:
Patient’s skin has solar changes with areas of hyperpigmentation, some keratoses, fine wrinkles and a dull appearance. The lower eyelids show the steatoblepharon (lower eyelid bags), together with the nasojugal groove and the malar groove, together with ptosis of the cheeks, which makes the groove below the eyelid bags look worse. Mild lower eyelid laxity on snap-back examination
Surgical Plan:
For the skin, we would do the fractionated CO2 laser and apply mesenchymal stem cells to the skin. After the laser, patient will use water-and-white vinegar soaks and apply our special cream which will speed up the healing of the skin. Patient understood that pinkness fades slowly and that after a month, we often use the Aerolase laser to make the skin look even healthier and youthful as the Aerolase Neo stimulates the formation of new collagen. For the baggy lower eyelids, the plan is to perform a transconjunctival lower blepharoplasty with fat transposition into the malar groove and the nasojugal groove, lateral canthal tightening and a pinch blepharoplasty
Surgery:
Bilateral lower blepharoplasty with fat transposition, pinch skin removal, lateral canthoplasty and fractionated CO2 to the full face with the application of mesenchymal stem cells was performed as planned. The patient was put on a Medrol dose pack and covered with anti-viral and antibiotic medication. Use of a fan on the face for the first two to three days after each soak of the face with water-and-vinegar was advised. Patient slept with two to three pillows for three nights.
Discussion:
The changes of the skin and lower eyelids were discussed with the patient and details of the proposed surgery and laser discussed.

Assessment of Results:
Nice tightening of the lower eyelids with improvement of the lower eyelid bags and good skin tightening. Very mild left lower lid skin laxity related to the left side of the face being smaller than the right side. The skin has improved immensely with the fractionated CO2 laser and the use of mesenchymal stem cells. Further improvement can be obtained without any downtime with the painless Aerolase Neo laser which builds new collagen.
Patient’s recovery:
Patient noted that the skin of the face was swollen and pink and peeled over the first ten days. Her pain level was 1 – 2/10 and only used Tylenol and extra-strength Tylenol for the discomfort. She was able to apply makeup by day 14 and socialize with friends and family. She had blurry vision with red eyes for the first couple of weeks but she was still able to drive and read.
Patient’s Opinion of the result:
I can’t believe I waited all these years with these hated bags. I feel much more confident and I enjoy putting makeup on my smoother skin. My friends all tell me I look like I am rested and they cannot tell that I had surgery.
Advice to patient:
She will need to use sunscreen when she is outdoors and we have put her on retin-A which will keep her skin healthy. She will start monthly treatments with the Aerolase as she would like to see further improvement in the skin. She understands that the body leaks fluid when we are tired, after salty meals, etc and that mild swelling of the lower eyelids will occur. However, the bags will not come back the way she had them. We have asked her to use vitamin C serum on the lower eyelids once at night.
